The most serious clinical expression of osteoporosis is fracture. Despite the availability of a number of drugs capable of reducing fracture risk by over 50%, the use of drugs to prevent bone loss remains inadequate.

Bisphosphonates will remain the gold-standard approach to osteoporosis treatment due to familiarity among prescribers, genericization of leading brands and innovations in dosing among novel products. The osteoporosis pipeline is top-heavy, with 50% of drug candidates in Phase III or higher.
